Client Consent for Reiki/Healing Touch and Hypnosis Sessions

Have received information and understand that Reiki, Healing Touch and Hypnosis are gentle complimentary, energy-based approaches to health and healing that can assist my body in its natural ability to heal. I fully acknowledge and understand that this is accomplished through the use of contact, and non-contact touch along with voice guided support and instruction for stress reduction and relaxation.

It has been explained to me that these modalities are not intended to replace any currently prescribed medical treatments as prescribed by my physicians nor any other medical or mental health care that I have been advised to seek by my medical doctors or other licensed health professionals.

I have been informed that my practitioner, Christen Resmo, RN, BSN will neither diagnosis nor prescribe for any condition that I might have, nor does she make any definitive claims regarding specific results for the Reiki, Healing Touch or Hypnosis treatments that I receive.

I have been informed that Christen Resmo, RN, BSN, HNB-BC, CHC, CRMT, CCH is not licensed to practice medicine in the State of Colorado, or any other state. I have been encouraged to consult a licensed medical provider for any physical, emotional or mental complaints that I might have.

Some of the benefits of Reiki, Healing Touch and Hypnosis include, but are not limited to:

• Reduction in pain, anxiety, and stress (both physical and emotional)
• Decrease in nausea, tension and heightened emotional states
• Preparation for medical treatments and procedures to reduce fear and feel mentally ready
• Support during chemotherapy and radiation
• Support in the body’s natural ability to heal
• Increased stress resilience and physiological coherence

I have been informed that all information and records are treated in a confidential manner. My experience during this session are confidential subject to the usual exceptions governed by State or Federal Laws and regulations.

My questions have been answered to my satisfaction regarding Reiki, Healing Touch and Hypnosis and Christen Resmo’s background as a practitioner, and what I might expect from this session.
